In order to be registered for the camp, we must receive ALL of the following:
- Completed Registration Form
- Immunization Record/Physician's Notice
- Emergency Liability Release Form
- Release of Liability Form
- Copy of Physical (can not be more than 2 years prior to July 18th 2012)
- Full Payment
